Service Request forms

HBCF is 100% funded by our local community to support Hunter residents diagnosed with and undergoing active treatment for breast cancer in the Hunter region. Please complete the relevant online form below. If you are not a Hunter resident, please contact your local service provider. 

HBCF currently defines active treatment as chemotherapy, radiation and breast surgery. Our clients are eligible to access services for up to 9-months from their referral date to HBCF unless the client has received an Advanced or Metastatic diagnosis, in which case services can be accesses ongoing as needed. 

We respect the individuality of each journey and are committed to ensuring our clients receive the support they need. If continued support is required HBCF will make the necessary referrals specific to the clients needs. Our client’s well-being is our top priority.
